2. Fill out the form and submit.

3. Wait for your Email from Lactation Network confirming your benefits. Don’t worry, they are speedy at getting back to you!

4. Once you receive confirmation of coverage, click the “Book Now” button below and choose your services.

Payment is due at time of service or upon booking appointment unless covered by Lactation Network in advance: All major credit cards, Venmo and HSA accepted.
